Contractor Registration

PLEASE NOTE:  The State of Illinois requires contractors, property managers and others paid to replace windows or renovate residential houses, apartments, and child-occupied facilities, built prior to 1978, to be certified by the U.S. Environmental Protection Agency (EPA) to protect children from leaded dust that may result from disturbing lead-based paint.  For more information, please call the Illinois Lead Program at (217) 782-3517.

All contractors doing work in the Village of Streamwood must be licensed by the Village.  An application for the license is available in the Forms and Applications page in the Reference Desk section of our website.  Contractors must also submit a completed Consumer Rights Acknowledgement Form, signed by the owner.

2. City of Chicago License (if not State licensed)
3. State of Illinois Registration number
4. Cross-Connection License (copy)
5. Fee - $50.00 administrative processing fee

 Sub-Contractor - Electrician

1. $10,000 License and Permit Bond (Original)
2. Certificate of Insurance (Original)
3. Electrical License or Supervising Electrician's Card (copy)*
4. Fee - $75.00
