Streamwood Police Investigate Fatal Traffic Crasj

The Streamwood Police Department, with the assistance of the MCAT Serious Traffic Accident Reconstruction team (STAR), is investigating a traffic crash which occurred late Sunday afternoon.
 
Streamwood Police were called to the area of Illinois Route 59 near the intersection of Tallgrass Ct. at 4:07 pm for a crash involving a 2005 Honda CRV and a private ambulance operated by the Trace Ambulance Company.

A preliminary investigation reveals that the Honda CRV was traveling south on Route 59 when it crossed into north bound traffic, where it was struck by the ambulance.
 
The three occupants of the ambulance were transported to St. Alexius Medical Center in Hoffman Estates with non-life threatening injuries.

The two occupants of the Honda were likewise, transported to St. Alexius Medical Center where the driver, 64 year old Jitendra Shah of Hoffman Estates, succumbed to injuries received in the crash. Mr. Shah's 53 year old passenger remains in serious condition.
 
Weather and road conditions may have been a factor in the accident.
